Option 1: Download the application below, then:
- Complete all required information on your device
- Read and sign the Participant Rules and Regulations
- Read and Acknowledge the Lighting Requirements
- Print, Read and Sign the Indemnity Agreement in front of a Notary Public. **You’ll need to bring this to the mandatory meeting on November 29th)
- Email the completed application to us at miraclesparade@gmail.com
Make sure to send your application fee through our PayPal link at the bottom of this page.
Option 2: Use the link below to PRINT the application, then:
- Complete all required information
- Read and sign the Participant Rules and Regulations
- Read and sign the Indemnity Agreement in front of a Notary Public
- Read and Acknowledge the Lighting Requirements
- Mail or drop off your completed application and a non-refundable application fee to:
